- Birth certificate for all grades (official birth certificate, not hospital certificate)
- Physical completed by a licensed physician dated no earlier than August 1st of the prior school year
- Immunization record (Blue Form) *
- Proof of residency, can be in the form of:
- Rent receipt - Home contract
- Utility bill with parent(s)/guardian(s) name
- Notarized statement from family member with which they reside
- Residency Verification Form
- Copy of last report card if attended school previously or name an phone number of school previously attended.
